Ankle-brachial index measuring device (ABPI MDTM)
Motivation
Peripheral arterial disease (PAD) increases the risk of a heart attack or stroke. Peripheral arterial disease is a serious condition caused by insufficient circulation in the legs. The arteries of the lower limbs are narrowed by atherosclerotic plaques – cholesterol accumulations in the arterial walls – which restrict the flow of oxygen-rich blood.
The disease can occur in all arteries of the body. Almost 60% of patients with BAP have blocked arteries in the legs, as well as in the heart and brain.
The ankle-brachial index (ABI) should be measured in primary care as a method of diagnosing and monitoring BAP.
The automatic device for measuring the ankle-brachial index makes it possible to diagnose patients faster!
Ankle-brachial index scale:
- 1.41 or more – incompressible;
- 1.40-1.00 – normal;
- 0.99-0.91 – at the limit;
- 0.90 – 0.41 – abnormal;
- 0.40 or less – serious.
The normal value of ABI is between 1.00 and 1.40. This shows that the blood pressure of the lower limbs is equal or slightly higher than that of the upper extremities. The ankle-brachial index measurement performed is a safe and faster alternative than the Doppler probe measurement.
Arteries obstructed by atherosclerotic plaques cause the ABI to decrease, therefore the examination is used to determine the BAP. Therefore, the evolution of the disease is reflected in the change of ABI.
A person can be symptom-free and still have BAP. Indeed, the symptoms appear only if the stenosis of the artery exceeds 70% of the vascular lumen.
Even though BAP cannot be cured, symptoms can be limited with appropriate treatment. That is why it is essential to measure the ankle-brachial index periodically to detect any significant decrease in it
Only 25% of patients with BAP are undergoing adequate treatment for their disease. This low percentage is largely due to the fact that the general public does not know about this disease.
How does the Solution work?
The device for measuring the ankle-brachial index has two modes of operation:
The primary mode is for measuring the ankle-brachial index, when you will place the cuffs on the arm and on the left and right ankles. The measurement results 8 are the left ankle-arm index (LEFT ABI), the right ankle-arm index (RIGHT ABI) and the blood pressure in the arm (SYS, DIA).
The additional mode is for the independent measurement of blood pressure in the arm. In this way, you will only be able to measure the blood pressure in the arm, so you will only place one cuff on the arm. The result of the measurement is the blood pressure in the arm (SYS, DIA).
Step 1:
Connect the device to the mains using the AC/DC power supply.
The power connector is located on the back of the device and is labeled “5V DC”. Connect the cuffs to the device by inserting the tube of each cuff into the same-colored connector on the device.
Step 2:
Turn on the device using the ON/OFF button. The main screen will appear on the display.
It is not necessary to change the configuration when using the basic functions of the device. If you use the device for the first time, the language, time and date menu will appear. To set the menu correctly, see point 5.1.
Measurement of brachial tension index at the ankle
Step 3:
The patient must lie down and remain still. Place the cuffs on the arm (cuff labeled “ARM”), left leg (cuff labeled “LEFT ANKLE”) and right leg (cuff labeled “RIGHT ANKLE”). Observe the following colored markings:
- RED > left or right arm
- YELLOW > left leg
- GREEN > right leg
Place the cuffs so that there is a distance of one finger between the member and the cuff.
The cuff on the arm must be positioned so that the label for the artery (ARTERY) is placed in the direction of the elbow. When the cuff is positioned, observe the image below. The tube of the cuff must be directed towards the fingers.
The ankle cuffs must be positioned so that the ankle label (MEDIAL ANKLE) faces the inner part of the ankle. When you position the cuff, notice the image below.
Step 4:
The patient must lie motionless in the dorsal elbow position. When you are ready, press the START button. The measurement will start. The patient must not move during the measurement. The whole process takes about 1 minute.
During the measurement process, the current blood pressure values will be displayed on the screen, as well as the pressure oscillation graph.
Step 5:
When the measurement process is finished, the results will be displayed on the screen. The ankle-brachial index is displayed in green, yellow or red, depending on the value. The color scale is taken from “Guidelines for the Management of Patients With Peripheral Artery Disease” published by the Journal of the American College of Cardiology.
- Below 0.89 – red
- Between 0.90 and 0.99 – yellow
- Between 1.00 and 1.40 – green
- Above 1.41 – red
Step 6:
Return to the main screen using the HOME button. From this point, you can observe the measurement history and change the settings.

Blood pressure measurement in the arm
Step 3:
The patient must sit comfortably on a chair, keeping the back straight. Only the red arm cuff (labeled “ARM”) is placed on the patient’s upper limb). In this way of working, the green and yellow cuffs are not used.
Position the cuffs so that there is a distance of one finger between the member and the cuff.
The arm cuff must be placed so that the label for the artery (ARTERY) faces the elbow.
When the cuff is positioned, observe the image below. The tube of the cuff must be directed towards the fingers.
The forearm must rest on a straight surface. The cuff must be at the same level as the patient’s heart.
Step 4:
Press the blood pressure measurement button (BLOOD PRESSURE MEASUREMENT) in the independent blood pressure measurement mode.
When you are ready, press the START button. The measurement process will begin. The patient must not move during this time. The whole process takes about 1 minute. During the measurement process, the screen will display the current blood pressure and voltage.
Step 5:
When the measurement process is finished, the screen will display the blood pressure measurement results in the arm.
